The report in question - the report name or the website address for the report
How the report was run - what screen was the report run from, was it run manually or as part of a schedule
if it was run from a schedule, support also need to know which schedule it was
The Criteria used to produce the report - we will need to replicate the report so we will need to know what filters were used to generate the report.
Specific examples of the problem - for example the ticket number that is not appearing where it should, or the specific entry on the report that is incorrect.
Frequency of the issue - does it happen every time you run the report or only when using these specific criteria?
